If I had to make a guess about how c!Tommy's story ends (if Tommy decides on ending it in an angsty manner), I'd say c!Wilbur dies as he knows that he doesn't belong into the world of the living anymore and c!Tommy has a breakdown resulting from that.

Although c!Wilbur managed to free c!Tommy from c!Dream's clutches by making c!Dream believe that he destroyed the Disks as a way to redeem himself and to set up a better life for his brother, he underestimates how much c!Tommy needs him.
So when c!Wilbur decides that he's done in this world, c!Tommy finds out about it, and receives the news very poorly...
And he begins to question himself... and the point of his current existence.

He has to wonder, what's, or rather who's, left on the server for him?
Who is there to spent a happy life with, what does staying there accomplish?
C!Dream said that c!Tommy bound together everything, making things matter to people, so wouldn't that mean that his presence is the key to c!Dream's power?
Wouldn't this mean that he could end everything, could undermine c!Dream's control, if he just... left?
Followed c!Wilbur, permanently?
Because if there's no place in the world for c!Wilbur because he already died, then there should be no place for c!Tommy, either, anyways.

Not that any possible conclusions that may come from this train of thought would be easy to execute, given that the Revive/Revival Book still exists...
But you know, if there's anything that c!Tommy has learned anything over the years, it's how to fight back and persist.
There are always ways around and powers beyond that troubling book...
