When things go wrong, we love to find someone to blame, but the blame game ignores a fundamental truth: mistakes are inevitable.

The alternative? A "just culture".

Devised and championed by leading academic and part-time pilot Sidney Dekker, just culture is an idea embraced by an impressive array of organisations and industries, and it represents a fairer and more effective way to deal with and learn from our mistakes.

How does a just culture work? Find out in this explainer video from iluli by Mike Lamb – explaining the ideas and innovations changing our lives.

Great video. The video didn’t mention this, but fostering trust is essential for building a just culture because it enables transparency, honesty, and accountability ~ where employees feel safe and secure, and are encouraged to speak up without fear of retaliation. If Dave doesn’t trust his leadership, he’ll likely act in fear, and everybody loses.

And, if his leadership had once fired a coworker for an honest mistake after 10 years of excellent work, Dave is going to have a hard time trusting his leadership forevermore.

Very informative topic Iluli as usual i want to say 2 things 1 is about the video and the other is more of personal not: I really liked the video and mistakes do happen but sometimes corruption is involved i think it is better to state that point and also really when a group is more open the group as all is more responsible. My second thing i wanted to say that i really want to support the channel by sharing my knowledge, I have researched alot on multiple topics and i really want to support the good work you do on Youtube and i wish to go further with you as i believe in your channel. Now i cant give you money as im not in a position to give nor expect to receive them but knowledge is more important than everything i hope this finds you well

This is a great video, but it just seems impossible nowadays that Just Culture would ever be implemented rather than people just playing, as you said, The Blame Game. It's much easier to do than actually think about the reasons of course, so why dig deeper when you can just blame Dave?

I do hope this video gets aa popular as your previous ones, this idea needs to be heard by everyone.

Ah yes, this is hit on personal level. On higher management, the fear of failure, and scapegoating can be a life-threatening for everyone. Especially in the rat race environment.
In the middle management, they're literally squeezed both body and soul.
I had no idea that luck really counted when growing up, and learned that hard way to realize.

This is a thought-provoking video, but as much as I agree with many of its points, including how counterproductive the "blame game" can be, I'd also like to offer a counter-argument: even if the incident was not the fault of the person who pressed the button, but was caused by a number of factors, those factors don't arise out of thin air. There IS usually a person who is responsible for managing test data, for example, and making sure that testing doesn't impact actual customers in any way. Likewise, if the company hierarchy is structured in a way that causes people to make more mistakes, there should be a person who has (some) responsibility for allowing that problematic situation to arise. Of course, it's a separate and valid problem that such people, who are usually higher up the food chain, rarely face any punishment - it is much easier to find a scapegoat, like poor Dave.
