There are many variants of this test - all of which test for peritonitis. There is the supine position tests (which this video covers) and also a standing variant in which a patient stands on their toes and drops to their heels (effectively creating the same disruption to the peritoneal layer). It shouldn't be used to definitively diagnose peritonitis, but it is still a valid clinical test. Google "Markle sign" or "heel jar tenderness" for any verification!

I did this with my last abdominal pain pt. The nurses at the ED had no idea what I was talking about when I told them the pt was positive on the heel jar test. I explained and they said they had never heard of that. Made me kinda nervous.

Umm yeah, as a surgeon practicing for over 12 years please do not use this test...its a cruel way to elicit peritonitis and doesn't come across as very professional or respectful. There are other methods that you can talk the patient through. Their car/ambulance ride to the hospital will suffice.
