I work in survey design. The second-to-last one might make sense if we saw the questions. Because people who are paying attention will notice the change, but people who aren't paying attention will select all the answers in a column. This presents an issue where if one questions is framed negatively and one is framed positively, then someone who wanted to select all positive answers might unintentionally select some negative ones. For example: "Did you enjoy the show?" "Do you think the acting needs improvement?" Each question has a different positive answer, being Yes and No respectively. So in this context, it may make sense that Yes and No would be swapped.
