The crude ladder going down @ 0:41 is not on the Sphinx's head, it's going into a hole at its rear, left hind quarter, at ground level.
The opening on top of the Sphinx's head is not a secret, as can be clearly seen on the old black and white aerial photo. Which shows no hatch present, as this was added later.
