Cemento osseous dysplasia :

• It is a group of fibro osseous leison and

most common

• Occur in tooth bearing area of the jaw bone

• Form excessive cementum like mineralized matrix

Etiology and pathogenesis still remain unknown

. Mostly due to reactive or dysplastic processBecause these leison formed from proximity of the tooth

Many investigators says

Formed from PDL

Defect in extra ligamentary bone remodelling

WHO finally classify:

1)focal cemento osseous dysplasia

2)peri apical cemento osseous dysplas

3)Florid cemento osseous dysplasia

Focal COD:

• Common in middle aged females

• Especially African-Americans • Mandibular molar frequently affected

• Majority leison Asymptomatic • Average leison 1.5 cm in size

• mis daignosed as peri apical infection so pulp vitality test is done
Peri apical COD:

• Predominantly involved in apical region of vital mandibular anterior teeth

• Frequently more than one teeth

• Either as solitary or multiple small leison

some Time exceed in 1 cm in diameter

Leison seen in black people

Asymptomatic

• Self limiting

• Sometimes expand the bone
Florid cemento osseous dysplasia :

Common middle aged black women although asian may be affected

• Asymptomatic

• Multifocal involving both maxilla and mandible especially posterior region it is bilateral leison

• Dull pain draining sinus associated with exposure of sclerotic bone cause alveolar atrophy

In denture wearing patient

Complication of tooth extraction
Histology : -

Highly cellular

• Numerous island

Fibroblast

Hemorrhage
Treatment

Self limitating Extraction /biopsy should be avoided that leads to osteomyelitis
