Seth MacFarlane, himself, everyone IS able to understand Stewie fully well. However, because he's a toddler, they don't take him seriously and characters such as Peter and Lois just choose to ignore him.

The basis for this shtick is adults don't take the things their kids say seriously, so they tend to overlook or outright ignore practically everything Stewie says in spite of the fact he is speaking clearly, concisely, and is perfectly understandable

The only times when Stewie is actually understood by anyone other than Brian, Chris, or his baby/toddler-aged peers is when it's to make a joke or for the sake of the plot, such as how Stewie is often understood perfectly by characters outside the family or the main and/or regular cast
