Fluid responsiveness assessment is a challenge in itself. I use these following methods for assessment:
1. Passive leg raise test (sensitivity and specificity both above 90%)
2. Pulse pressure variation in mechanically ventilated patient
3. Stroke volume variation (I rarely use it)
4. IVC responsiveness.

There’s a JAMA article from 2016 that explains all these 4 methods beautifully. Hope this helps.

PS: Your videos are such immense source of knowledge. I’m literally binging on it. Keep ‘em coming, sir

Thank you for clarifying. Till now I knew that giving 500 to 1000ml bolus and looking for a rise in the BP. Unfortunately CO is not being monitored in my ICU.

Hello sir
patient shock on vasopressor support
Patient was fluid responsive, then at what rate fluid correction should be given?

Also in a post op surgical patient with pulmonary edema, with shock and severe anemia ( Hb 3) also cvp less (negative), after correcting pulmonary edema . Should the anemia be addressed immediately or can gradual correction of anemia be done?

So thats why my intensivist ask me to give a bolus of 200-300ml fluid and watch for bp and urine out put, whenever bp or urine output drops .
