The Urgent Community Response

The new Urgent Community Response service can intervene within two hours when elderly patients deteriorate, helping them stay at home, when it is safe to do so. The team, part of Oxford Health NHS Foundation Trust's community services, ensures that patients get the care they need at home, preventing unnecessary hospital admissions.
