The original rumors (at least the earliest mention online) was that Stephanie was 14 at the time and Vince found out a few years after the fact in about 1994. As the rumour was repeated the stated facts got muddled as all happening in 1994 (when Stephanie was 17).
I've been searching for the original post but it seems to have been lost from the internet. The original was from a forum/message board active around 2001/2002.

Probably the most straight up answer to this one that I've heard and the one that seems the most likely.
Bottom line: It sounds like Randy was a little too friendly with Stephanie. Possibly not to the point they got it on, but enough to get people noticing and get Vince aggrevated and suspecting something was going on, possibly egged on by a jealous individual(s) whispering in his ear e.g. "Hey Vince, I heard Macho bragging about doing stuff with your daughter. I wanted to keep it quiet but if it was my daughter, I'd want to know." The jealous individual(s) could have been any number of guys who had a grudge against Randy or simply stood to benefit if he was taken down a peg in terms of increased exposure for themselves. God forbid, it could've even been Stephanie herself looking to get a kick out of taking a big strong man like Randy down. I'm not accusing Stephanie of being that kind of girl, but believe you me sociopathic girls like that do exist. Either way, Vince being Vince, he probably confronted Randy about it man to man and demanded an explanation. Randy likely denied having sex with his daughter (quite possibly being honest). Vince didn't believe it and came to believe that on top of having a dalliance with his daughter, Macho was a liar to boot. Hence Vince developed a severe distaste for Randy that lasted the rest of Randy's life, deserved or not. In short, there was likely fire making the smoke, but it may have been much smaller than people believe. When all was said and done, the real victim in all of this was quite possibly Randy who ended up blackballed from the World's biggest wrestling promotion even after death, quite possibly for a crime (if getting it on with an attractive 18 year old woman is a crime) he didn't commit.

I feel like this podcast will get Dutch working for a major promoter again soon. Its really exposed him to a new audience of people and his knowledge of the business is exceptional

I'm watching this and all I'm thinking is "How cool would it be if Vince, now supposedly out of the business, would start getting on podcasts and share the details and his own views on so many wrestling stories involving him or WWF/E. Hear him speak openly as Vince McMahon the man, not the chairman or the character but his real self. Just like Austin or Foley or Taker are doing now that they are no longer active".

I've read somewhere that McMahon was angry that macho Man took the slim jim deal to WCW. According to King Kong Bundy McMahon got rid of Bundy because he did a commercial in Europe without his permission. I know this sounds crazy but McMahon is an unusual individual. He might hold a grudge because he took the slim jim deal with him and it might not have anything to do with Stephanie

The hall of fame rumor is just silly at this point. There's a couple sources on it but most importantly Lanny has laid it out. Randy wanted to leverage the HoF induction to be his family because he wanted to honor his father. It was Lanny who ultimately agreed to let them induct Randy.

Vince’s farewell speech he gave to Randy on an episode of Raw leads me to believe that this rumor is just that. So he never came back because Vince took Randy going to WCW personally. Vince didn’t anything like that for anyone else.

I want to know how often Stephanie was hanging around behind the scenes at that age for Randy to pursue anything. Those guys were on the road 24/7 I don't see where Stephanie would have been around "The Boys" all that much.
