It is a byzantine relic. In Byzantine times they used to have a punishment of public humiliation in which the judge dived his hand into black ash and opened his palm to rub the face of the offender. So the person with the "dirty" face was considered disgraced. That is why this insult is called the "moutza" hand, which derives from the greek verb "moutzourono" which means "to smudge/black someone". Today indeed it is synonymus to the middle finger
