How It Happened?

The plane had taken off from Wichita, Kansas Airport
A PSA Airlines plane, operated by American Airlines, collided midair with a Military Black Hack Helicopter at around 9 p.m. Wednesday the 29th 2025 while approaching a runway at Reagan National Airport.

From the top Angle you can see Flight AA- 5342 was approaching from the South to Land at the Reagan Washington National Airport.

This is surrounded by the Potomac River Running around Pentagon city vehicle to the the North you will find the US Capithol Hill where US President Trump Inuguaration took place a 3 weeks ago.

Im a former Air Traffic Controller with ten years experience at one of the busiest military airports in the world during the end of the Vietnam war and several civilian airports afterwards. I was a tower and radar controller at Nas Miramar, San Diego, Ca. home of "Top Gun" at the time. I was fully facility rated both in the Control Tower and Approach Radar and controlled every type and kind of aircraft known to man at that time.
The ATC rules and regulations that controllers apply to air traffic are written by the FAA (Federal Aviation Administration). One such rule as briefly explained in this well done video is the difference between IFR (Instrument Flight Rules) and VFR (Visual Flight Flight Rules). This is very important to understand. VFR simply means the weather is clear enough for the pilot to fly visually his aircraft and maneuver his aircraft, at will, to avoid other aircraft in his area. The pilot may not fly into the clouds or in reduced visibility. The VFR aircraft is not under positive ATC control, they can get traffic advisories if they ask ATC for it. Some pilots do, some don't. IFR means the pilot is qualified to fly in the clouds on instruments and files a "flight plan" with ATC before take off. IFR aircraft are under positive ATC control from the time they take off until they land. All commercial passenger aircraft are on IFR flight plans and provided ATC separation per the rules and regulations set by the FAA.
When I first heard about these "corridors" along the river while commercial aircraft are on approach to that airport, I was appalled! Im sorry, a couple hundred feet crossing an active approach end of a runway with a commercial jet on short approach is inherently dangerous! Reports say this helo was at 400. With the city lights in the background and other aircraft taking off from the same airport, it would be easy to mistake what you think you see. Also I find the phraseology used by Air Traffic Controllers appalling these days and certainly in this occasion. I never heard the controller say "Pat25 Traffic 10 O'clock 1mile commercial aircraft on final runway33. Or whatever the position and distance was and keep repeating the closing distance till the VFR helo turned.
It's often said by us that were in the ATC business, that the rules and regulations, are written in blood by the FAA. I don't mean to sound insensitive to the victims and families of this tragedy. These are the rules the FAA has instituted and most times it works quite well. However, I'll bet you see some rule changes the FAA will institute regarding VFR aircraft crossing in front of and underneath landing aircraft.

Former Blackhawk pilot here…that chopper has an onboard system called a FVDNS (Full View Digital Navigation System). In simple terms, an FVDNS is essentially a 360 degree radar that surrounds the hawk in a 1/2-3/4 mile bubble of digital display radar. Meaning there is a giant 24.5” LED display right in front of the pilot that shows any object in full within that bubble. With clear conditions (no fog, snow, heavy rain etc). Could show clearly up to a full mile, but half a mile to be safe. That display will show the pilot what the object is, how big it is, how fast it is going, the direction it is heading & most importantly…how close you are to the object and if a possible collision is imminent. The ONLY way this could have happened judging by the footage, is if the pilot either had the system manually overridden and switched off (totally against regulations…some pilots do do it however when flying over wide open spaces because that screen is bright and can cause eye fatigue even through visors). Flying over a city, right over an airport is the exact spot to have it ON and pay full attention to it. Which brings me to the only other option, the system was on, functioning properly, and the pilot simply disregarded the system and flew by line of sight. Some blackhawks even have systems that will override the pilot (in case of a health emergency/loss of consciousness etc.) and evade the danger on its own. This model obviously didn’t have that unfortunately…it’s a feature usually only put on hawks going in to active war zones. But this pilot was flying in the dark, above an airport…NOT paying attention to instrumentation. That alarm should have been blaring for 10-15 full seconds at least before impact. I picture the pilot frantically looking around to see the obstruction (rookie horrible error…Blackhawks have notoriously poor visibility, which is why the panel is so important) when they couldn’t see anything they kept the status quo and continued on the steady course…right into a passenger jet. I still have yet to wrap my mind around this. Looking forward to more info coming out.

I’m an Army veteran my job was a UH60 crew chief/ mechanic, I don’t understand how this flight path crossing situation was allowed to be so common?
No way a flight path for military helicopters should be allowed to cross a landing flight path.
There should have been a separation corridor for all aircraft on an approach or departure vector at any altitude below 2500 feet.
All other aircraft should not be allowed to cross these corridors to avoid any collisions.

A couple of my friends who were attending the US figure skating national development camp were aboard this flight.
This video was especially helpful for people like me who knew some of the victims on a personal level. Everyone in the USFS community is still really sad about the sudden tragedy, but I feel that this video will help us all understand what had really happened to our skaters, and the other victims.
I was really impressed with how well you made this; the quality of the animations, the explanation, and how fast you were able to get this out there. This was amazing!

Retired USAF officer here, my last seven years was with base level IG. I wrote, evaluated and debriefed major accident response (MARE) exercises, among other things. I would have loved to have had graphics like this to work with. Well done AiTelly.

I have 41 years of experience in air traffic control and I can affirm that it is not good practice to allow unrestricted operations to any VFR aircraft in dense traffic at critical moments of arrivals and takeoffs! It's dangerous!

For the life of me, as a guy who works with Air corridors for the US Army. I still don't understand why the Blackhawk went from 200ft elevation to 300ft elevation before the crash in an area where 200ft was the ceiling in that corridor.

00:48 Wrong - the helicopter collided with the jet because the helicopter had the responsibility to avoid the jet. PAT stands for Priority Air Traffic This is the problem though, thinking you are the priority when you are not. FAA and Pentagon Officials who approved PAT traffic in this area should stand trial for manslaughter over this. Not matter how long they have been retired. Unaccountable government must stop.
