Mimir: “He tortures me, you know. Every day, brother. Odin himself sees to it personally, and believe me, there is no end to his creativity. Every. Single. Day. This... this isn't living.”
The most tear jerking moment in a game I’ve ever felt. Mimir was so open hearted all the time but in his most desperate of times he revealed the true hell he was going through.

The girl that was paralyzed from a potion and had to watch/feel rats eat her alive only to become a heartbroken wraith in the Witcher 3 had a pretty horrid death imo.

Honorable mention: the flood from halo, the people who get turned onto flood monsters are still conscious and alive during the whole process of their organs moving around in their body and their head and neck being ripped back behind the back for the flood monster to take control of their brain and basically have a psychological war with its host as the host feels literally everything going on

The thing you forgot to mention was the head crabs reversed dialogue that is “oh god help! Help me” and when you light them on fire “oh god I’m on fire! Why why why why why!” It’s pretty messed up

What about the boy ghoul in Fallout (4?) where he, an immortal child, was trapped in a refrigerator for 200 yrs unable to die from starvation, dehydration, and suffocation.
Constantly calling out for help that never came, waiting trapped alone in the dark, in a confined space, wanting to see and be with his family (mom and dad) again.
You had the option to help him and reunite him with his family that was also still alive from ghoulification or sell him.

Harold’s fate was worse than just being turned into a tree. He had a tree grow out of him. He was exposed to the FEV and ended up becoming Ghoul-like from it, rather than the usual crazy buff super mutant. Over time the tree grew through and around his body, pushing some parts, like his heart away. So he spent years slowly getting his body split apart by a tree that grew to encompass nearly his entire body, trapping him in the sedentary and painful tree life. Poor guy.

Edited since he's technically not a ghoul.
