Onlookers scramble to safety as ship crashes into downtown San Diego dock

An investigation was underway into what caused a cruise ship to crash into the seawall near its dock along the downtown San Diego water front Thursday, injuring three passengers.

I think the problem is that your horn is connected to the throttle and given it's name the horn blower whilst blowing it's horn is inadvertently activating the throttle which is why all the "horn blowers" seem to be having this issue.
