I've had to get a metal rod in my collarbone after it broke and i gotta tell I wished they would've just made a big cut like he's describing.
They did those small incisions for me and the one on my chest got so irritated when they pulled the metal rod out that it got swollen really bad and eventually infected aswell. (Not from the operation itself though tbf)

So instead of having a somewhat cool looking scar across my shoulder I now just have a huge patch of scar tissue in the middle of my chest.

Surgeries are very often done by residency/specialist students while being heavily supervised by the consultant especially if you get surgery done in a university/public hospital
