The Stanley Parable should be on this list. There's nothing quite like being taken to The Serious Room and having the narrator of the game berate you for trying to cheat, and being sentenced to a hundred billion trillion years of standing in The Serious Room with only a very serious-looking table for company to make you think about what you've done.

Alien Isolation didn't fiddle around with tricky punishments, it just straight up didn't let you use the keypads until you've found the keycode for it

The first one can be avoided by just loading back a save, losing once, and then redoing the whole thing. Unless it also programmed in a way to detect that you just so happened to lose one roll before the 20 roll limit.

The Witcher 3 had a good one, make too much gold too quickly through a glitch or exploit and Geralt gets a visit from a suspicious tax man asking about undeclared income

If I recall correctly, some of these, like the fable frying pan and the dungeon master's ire one also trigger if you figure it out without collecting all the clues (because the game only checks the "has found all the clues?" flag).
Fable at least, powers up the pan a little for each clue you find, but I think Prey had a pass/fail thing (so, if you find three digits, but brute force the fourth, you still get the DM's Ire)
