They know their way around the different, sometimes complicated systems. They’re also trained to spot inefficiencies. Care coordination can be life-changing. Especially for those experiencing multiple health challenges. However, the need for care coordination is great. And some community members still fall through the cracks.

Our vision is a stronger and more seamless system of care coordination in Southwest Washington.
