I recognize all of those names, and I've been required to use two of them. They are absolute nightmares to work with, and I've had to have multiple doctors fight tooth and nail with them to get medication that I need to function covered

It's almost like there should be a nationalised list of covered drugs that's managed by an independent government agency driven only by the science. A Pharmaceutical Benefits Scheme which, if a drug is on there and used for its intended purpose, should be subsidised to buy so the public doesn't have to pay outlandish amounts to stay alive. I dunno, just spitballing here 🇦🇺

It also works both ways: The PBMs say to the manufacturer "give me a better rebate or we'll take you off the formulary."

The same thing happens with insurance companies and hospital healthcare prices. In order to justify the membership benefits of the insurance carrier’s provider network, they offer steep discounts on health care services, in order to be able to provide steep discounts on their services, health care providers must artificially inflate the prices listed on their chargemaster (their registry for the cash prices they charge for every service and supply/material they can do).

So yes, the problem is PBMs, but only sort of. The problem is that private for-profit insurance is the middleman to all of our health care.

these companies also make it harder for pharmacies to fill ppls meds - optum for example likes to incentivize mailorder instead of pickup and it takes like 30 mins on the phone to go around this…

If you currently get meds at cvs SWITCH all of your prescriptions to a local independently owned pharmacy if you are able. I had been using CVS but I get better service and no lines at my new pharmacy. It was pretty easy to do.
