Guess how 95% of ambulance companies make their money. Its not responding to emergency calls, its all in the Interfacility transfers or IFTs. Most counties have the crews and units to provide emergency coverage but 30-40% if not more are tied up on BLS renal roundups, hospital discharges, or other transfer calls. Even county based systems are shoveled into the overflow of transfer dogging at times. Pair that with the low wages, stress, and hours worked its no wonder no one wants to be in this field. Its a total systemic failure from the ground up. Ask the government to increase medicaid and medicare reimbursement on emergency calls and youll see ambulances spring up available for 911 like flowers in the spring.
