Hi Jennifer, thankyou for your clear and concise explanation. A tricky aspect for some of us to comprehend/appreciate is the initiation part of a change request. My experience suggests any stakeholder can raise/initiate a change request which is usually assessed by the PM (if it should be initially logged into the Change Request Log). Please confirm my thinking in terms of questions arising in the PMP exam
Secondly, if a change is entered into the Change Log (during the initiation/proposed change) is it recommended to add anything to the Issues Log (as well) which may directly/indirectly relate to the proposed change?

When should a change request be a problem report? I suspect that if it's not new requirements that are pushing the change, then it is a problem report.
