DoDD LOC Application - How to Video 3 - Significant Change of Condition LOC and Initial LOC

This video will walk you through how to use the Ohio Department of Developmental Disabilities’ online LOC Application; specifically how to complete a Significant Change of Condition Level of Care assessment AND how to complete an Initial Level of Care assessment. This video was designed for Service and Support Administrators in Cuyahoga County, although much of the content would be useful to SSAs in other parts of Ohio as well.
