As a therapist, she's probably very familiar with cynicism and sarcasm. She clearly knows he's being satirical, but she's also responding in a way similar to how she probably deals with sarcastic clients.

Art therapy can be very helpful. I did an art therapy group class when I was going through cancer treatment and it was a great way to deal with the stress and fears. I highly recommend it.
