I think most of the countries (including Pakistan) won't use Nuclear weapons first cause it is a guarantee of mutual destruction. So even if they have not officially said it but they will follow a No First Use Policy. This would also apply to China and India

US ex-secretary Pompeo quoted that Bharat and Pakistan were close to nuclear war post-Pulwama attack. I think if in future, Pakistan decides to throw nuclear weapons, we will use the first-use policy.

Vajpayee once time told if Pakistan uses nuclear weapons to destroy half of India, the next day there won't be a country called Pakistan. But I guess, even losing half of India is huge. So, yeah maybe future nuclear policy changes.
