South Africa and the USSR weren’t allies, the Apartheid government despised communism and the USSR supported the ANC.

Things that would be different:

-South Africa would definitely build another Nuclear Reactor in Pretoria by the Early 2000s.
-South Africa would be richer and more independent from foreign imports than it is today.
-The SADF would be the strongest army in Africa, with a fantastic Airforce, while the Navy lags behind.
-South Africa, like North Korea, would be a hermit state, always condemned by liberal media, but would use it's now vast nuclear arsenal as a deterrent against invasion.
-The death penalty would still exist for serious crimes and the Bantustans would be governed harshly.
-South Africa would form a buffer around itself, Botswana might not exist, Rhodesia might exist, Mozambique would be a second South Africa, Lesotho and Swaziland would likely be annexed and incorporated into the Bantustans, or their resoective Bantustans might be incorporated incorporated into them.
-Ethnic resettlement, European and American immigration and Afrikaners moving out of the cities into the countryside would reverse the low population growth among whites and eventually lead to a population of around 30 million by the 2030s.
-South Africa will not ally itself with Putinist Russia, they would be a superpower in the Southern Hemisphere, who would do business and trade with both the Russians and the West.
-The Chinese would be staunchly against South Africa, because ZA recognized Taiwan as the only China very early on.

TLDR;

If you are white, South Africa would be one of the greatest countries to live in with bountiful land, oppertunities and comfortable living.

If you're black, it is debatable whether or not things would be better, crime would be much lower, failing infrastructure like water, roads and electricity wouldn't be nearly as big of an issue as it is today, service delivery would be much better as the Apartheid regime wouldn't want you rioting, so they'll give you a good government and benefits.
