Challenging case of right coronary artery chronic total occlusion in a patient with prior coronary bypass graft surgery. Several challenges had to be overcome: (1) access site complication; (2) electrocardiographic changes and hypotension; (3) balloon rupture; (4) difficulty advancing equipment through calcification and tortuosity; and (5) losing guidewire position after successful crossing... Eventually successful recanalization was achieved. Persistence and humility are important for CTO PCI.
