Expenses in Usmle journey (Updated 2022)

Here we will discuss the total cost of USMLE journey for International Medical Graduates or IMGs from the time you start preparation of the USMLE exams till the time you Match in residency.

Along with passion and determination, a well laid plan and preparedness for the expenses at various stages is also vital in the USMLE journey.

To ease the discussion and understanding I will first break this down in a systematic way based on how much is the cost for each component of the journey like exams, USCE, applications etc. and towards the end I will classify it in the most practical way so that IMG’s can accordingly plan and prepare for their expenses.

This is my personal experience.
A high USMLE step 1 score has been an unofficial (sometimes an official) screening factor for US medical residency applicants. High scores on step 1 were seen as identifying the more desirable residency applicants. This was not the original intent of this exam. In 2022, the scoring of USMLE step 1 will change from a numbered score to pass/fail. Then the attention will turn to the USMLE step 2-Clinical Knowledge score. Since a primary factor of medical practice is clinical knowledge, the change shifts the emphasis toward the more relevant exam. Until 2022, a high USMLE step 1 score will still be important for US residency applicants.

Only you can decide if it is “worth it” to pursue a US medical residency position. It is costly, and you may need to borrow money from family members or a local bank. For those who match into a residency, most believe it is worth it. There are more IMG applicants than there are residency positions available for them. Applicants usually prepare to apply to more than one application cycle.

FIXED COSTS:

These are costs, in US dollars, that are approximately the same for every IMG-International Medical Graduate.

ECFMG application: $145

Medical school transcript translation: $250

ERAS token: $130

USMLE fees:

Step 1: $965 plus $160 international test delivery surcharge, total $1, 125.

Step 2 CK - Clinical knowledge: $965 plus $160 international test delivery surcharge, total $1, 125.

USMLE step 1 and step 2 CK are available at Prometric Test Centers in many countries. They have been closed because of COVID for the past two months.
