Here're the simple steps for Realistic Cracked Skin using Photoshop How to Create.
First choose a face you want to apply effect and then import the cracked texture over the canvas. And simply change the blend mode from Normal to Multiply. Shape the texture according to facial shape using Warp tool or Liquify or using both. Adjust the lightnesss or brightness of the texture using Curve or Levels adjustment. And finally you are done.
