In order to protect the ear from loud sounds, the sound has to first travel into the ear and get amplified by the ossicles in order to reach the hair cells and be interpreted as excessively loud. Only then will the stapedius and tensor tympanic reflex activate. You have to eat the poison first in order to know it is poisonous kind of logic.

Stapedial reflex lasts for10s and after that stapedial reflex deacys and regenerate effective if sound level70_100db above person's threshold.sound louder than it may cause muscle fatigue or during the period between reflex decay and regeneration loud sound reaches OHCs and can cause demage.

Damage to ear in loud sound - because the tympanic reflex(attenuation reflex) has a latent period of 40-80ms. So when a very loud sound traverses the external ear in a very short period of time(as in bomb blast) the attenuation reflex(tympanic reflex ) does not get to work there by damaging the ear.
