Tobias being a therapist plays so, so well into the stereotype of therapists going into the profession primarily to address their own foibles and neuroses. For someone who needs a high degree of empathy and verbal acuity, he's completely self-involved and oblivious. It's clear that psychiatry was, for him, not about helping others but to explore his own gender and sexual dysfunctions.
