I think it would have really happened if sasuke didn't leave konoha. Then Sakura would eventually realise that how much Naruto loved her and she would end up with Naruto. they probably would have been a better and much more close-knit couple than the canon ones. I imagine Sakura constantly scolding Naruto that while he is Hokage he enjoys playing pranks and evading his duties to be with his children. I guess Naruto leaving clones at work and seeing the real him staying home as much time as possible with Sakura-chan (he would find any excuse to visit her at work and admire her and her skills; like he does in Boruto by the way He would fill her with gifts and surprises for how romantic he is by character) and his family. I imagine Sakura always head of the medical department who as soon as she returns home finds children and a devoted husband who takes care of her and makes her feel loved as she deserves. I imagine a joyful, chaotic, large family (because in my opinion every quarrel would end with a fuck 😂). Children would be totally living earthquakes, both in character and in terms of power.
