To be fair, no legitimate therapy session should EVER be televised. As nice as this therapist was, she can’t make progress with Tammy as long as the cameras are there.

Tammy has no respect. This therapist is trying so hard. Tammy doesn't deserve the surgery. The therapist is still trying after the way Tammy treated her. Tammy is a narcissist, it may be her only way to protect herself, but the way she tries to hurt others in her life is a problem and even if you're not receptive to a therapist you don't need to be rude.

You can tell that Tammy is so embarrassed and she hates herself . She doesn't want help because she has a lot of pride as well combined with the embarrassment. She's a lost cause really. She's spoiled and she doesn't know how to cope or want to learn how because of her pride

Her problem isn't just her playing the victim it's her being unable and unwilling.

I really feel for the therapist, but sending the most obstinate and sullen person you can imagine to the peppiest and achieving anything other than highlighting the difference between them is almost laughable. Tammy will never lose the weight because she doesn't want to. It takes a lot of effort to maintain her weight at this point (let alone constantly gain), more effort than losing it.
