Surgery is the most important and major component of the management protocol. For colonic cancer enblock resection of the colon along with total meso-colon envelope containing D1, 2, nodes. Total or subtotal Cole Tony is reserved for synchronus tumors, polypoid disease or HNPCC. Adjuvant chemo is required for all stage 3 cancers and stage 2 with high risk features. Rectal cancers especially mid rectal and distill rectum (T2/3 or N1 onwards) require neo-adj chemotherapy or chemo-radiation prior to surgery. However MSI high tumors are resistant to traditional chemo and require immunotherapy for stage 3& 4 tumors.
Organ preservation, TNT and sphincter sparing therapies are some of the newest modalities.
