The tick has extended mouthparts that has backward pointing spikes (not screw like), which it implants into the skin (not the head) and uses to lock onto its host. It also secretes a cement like saliva which holds it more firmly in place
Tick Twister cradles the body of the tick and twisting (either way) helps break the bond it created with its saliva and the barbs withdraw from the skin, folding into the axis of the turn. This allows you to remove it cleanly, quickly and intact, without compression or distressing it. (Twisting using other methods like tweezers only increases the chance of its mouth parts breaking)
