Condolence to the family. As a Registered Nurse here in the US for 37 years (18+ years as an ICU/ER and have been a hospital administrator for the last 8 years), I can say that the nurse who restarted the IV didn't cause the positive blood culture per family's report. The IV had infiltrated and IV fluid had spread under the skin and when the nurse was unable to successfully insert the catheter into the vein, the fluid leaked out. The previous infiltrated area was evident by the swelling and consequently became purpled in color due to the lack of the patient's platelet count. The bleeding that was observed from the patient's mouth was also due to the low platelet count, she was very susceptible to bleeding. The patient's passing can be attributed to possible metastasis of the cancer. She may already have metastatic lung cancer and the only way to determine how far the cancer has affected the other organs is to have autopsy. The family has to provide a valid evidence that negligence was indeed the cause of the patient's demise. My own father died from the hospital's negligence and they know that I knew about it, but since my father had been diagnosed with terminal cancer due to pancreatic cancer and only had 3 months to live, no lawyers were willing to file the case against the hospital in Hawaii. The lawyers were aware that negligence was the main cause of my father's death, but the amount of compensation that would be paid would not be sufficient to even pay the legal team due to the number of days that my father had been diagnosed as terminal. I am truly sorry for the family, but as time goes, healing will come.

I may add, just because the nurse was not successful in inserting the IV catheter into the vein, it definitely did not cause a bacteria to get into the vascular system. Upon inserting the IV catheter, the insertion site leaked out the fluid from under the skin, there was already fluid that accumulated from the previous IV site. Most likely, the bleeding that occurred after the IV needle was inserted into the skin had added to the swelling because of the low platelet count. It was very important to place manual pressure to the former IV insertion site to prevent further bleeding, but it absolutely by any means did not cause bacteremia. The infection in the blood was not from the unsuccessful IV restart. When a patient's health is compromised either from receiving chemo therapy or simply dehydration, it is very difficult to start or perform a venipuncture. I've been considered as the best IV start nurse due to special technique that I've been taught by an anesthetist 36 years ago, but even me on rare occasions can not successfully start an IV due to patient's lack of vein access. Inability to start an IV does not make a nurse incompetent nor negligent. Majority of the nurses from the Philippines are considered by many hospitals and patients as excellent and caring nurses here in the US and other foreign countries, but unappreciated by our own country men. That's quite sad.

Putting blame on somebody is the easiest way to do. I greatly sympathize with the nurse. In a normal healthy person inserting a needle twice or thrice is not a big deal. We are afterall human beings who make mistakes. But blaming someone for the death of a person is a big deal and very traumatic for every healthcare workers. I strongly believe that justice be serve. We can not go on destroying one's name and credentials if we are not 100% sure that we are right. Heartfelt condolences to the bereaved family though. May she rest in peace.

I work as a nurse in a Medical Radiation Oncology Unit in Toronto, Canada. While I recognize the natural tendency to seek explanations and assign blame, it's important to approach these situations with a holistic perspective. Cancer, particularly when it has metastasized, is an aggressive and complex condition. The platelet count being very low and the weakened immune system are significant factors that contribute to the overall health of the patient.

Blaming the nurse SOLELY for the decline in the patient's condition may NOT BE FAIR. It's essential to recognize the multifaceted nature of healthcare and the unpredictable course of diseases like cancer. Acceptance is indeed key, recognizing the broader context of the patient's health condition and the challenges healthcare professionals face in managing such cases. It's a reminder of the fragility of life and the unpredictable nature of certain illnesses.

Immunocompromised patients are susceptible to different kind of infections. Even the simplest type of bacterial/viral infection can cause serious complications to the patient. Ang sakit lang na kulang sa information ang family to the point na isisisi sa mga kabaro naming nurses. Ang tagal ko ng nurse dito sa Pilipinas pero usong uso pa din ang mga ganitong klaseng pananamantala sa mga nagtratrabaho sa hospital. Ang bilis nila makalimot nung height ng pandemic na halos iaalay na namin ang buhay para sa nakakarami.

Condolence po sa family …Based sa info, leukemia ang Cancer niya… very hard to treat … I am a Nurse too and I work in Cancer Hosp in Leukemia unit. Napakahirap ng cancer na yan … most of our patient population cause of death is Sepsis… nagkataon lang na hindi na lagay ang PIV kasi hard stick ang pasyente kasi mababa ang plts and neutropenic pa and manas pa… pero hindi un ang cause ng death and hindi rin yun ang cause ng Sepsis… marami na siyang infection and nag multi organ failure na . Wag naman po sana isisi sa Nurse, maganda ang motibo ng nurse ang mag alaga ng pasyente… nagkataon lang po un . Sana ma explain sa family … ang praying for comfort sa family .

As a nurse I don't think the reason of infection and swelling is due to cannula insertion. Her platelet is too low. Praying for your comfort.

nurses dito sa pinas ay sobrang unappreciated, understaffed, underpaid, tapos lahat pa ng blame sa nurses pa. wag na tayo mgtaka na mga nurses ngayon mag a abroad na lng or mag shi shift ng career.

and di po kasalanan ng "bagitong nurse" as you described it po yung cause ng bacterimia nya. first of all immunocompromised mga cancer patients. bagsak immune system nila, bgsak platelet thats why konting galaw lang eh prone sa bleeding.

to that " bagitong nurse ", cheer up malalampasan mo yan. you did your best.

to that bida bidang nurse na papahiyain yung bagitong nurse na sa harap pa mag comment na bakit namaga, well sana hndi ganun ang approach mo. binibigyan mo lng ng anxiety yung watcher.

Ako po i just lost my mom last year lang pero sa ibang private hospital. I sympathized with your family, pero cancer po ang sakit ng asawa nyo sobra na sensitive ang skin nya. Kahit saan po sya tusukin, tendency po mamaga at magkapasa. Sa mother ko po since namamaga din ang kamay they transferred ung IV sa arm nya sa ibaba ng armpit.
Sa mommy ko nga since nasa ICU/CCU pa may nakakabit na pang bp to monitor kaya sobrang nangitim ang braso.
Kung nun umpisa pa lang po nagkaproblema na sa hospital na un, sana hindi nyo na po doon ibinalik, madali lang naman makakuha ng hospital record.
Sobra pong mahal tlga because its one of the best hospitals dito sa atin. Kun magiging problema ang pera sana sa ibang hospital na lang po dinala. Kasi po base on my experience quality naman po talaga at galing ng doctors, plus sobrang maayos na staff, magagalang at malinis.
Sa mom ko po hindi cancer pero inabot po kami ng 3M. Sigurado po mas mahal ang chemo dahil inpatient po sya.
How i wish na sana dyan ko din nadala mom ko.
Nabasa ko po mga comments, sana maelighten po kayo sa ibang bagay na hindi nyo po naintindihan.

Someone has to hold these peoples’ hand. Make them understand things because they are obviously mislead, misguided and confused. It is easy to put blame on other people especially to the “new nurse”. The woman has died from her cancer. Simple as that. I will pray for the “new nurse” that hopefully this incident won’t discourage her from further pursuing her career in nursing. I was once a “new nurse” and I can just imagine how traumatic this could have brought her. From a nurse to a nurse, mabuhay ka!!

Condolences to the family. How i wish well informed ang fam regarding the real prognosis ng patient. I also feel sorry for the nurse, pakatatag ka. Isa lang yan sa mga challenges na kinakaharap ng mga nurses lalo na sa Pinas.

That’s why kailangan talaga na ipaintindi sa family ang tunay na prognosis ng pasyente lalo na sa mga doctor. Kaya nga mas gusto ko yung prangka na doctor na sasabihin talaga ang worst possibilities na manyayari sa pasyente kesa magbibigay pa ng hope ma alam naman natin kung gaano kapoor ang prognosis. For sure kung naturuan ag napaintinfi lang sa kanila ng maayos ang case ng pasyente sa family baka makakaintindi rin sila. Kahit wala namang malpractice sa nagawa basi sa istorya na isinaad nila, hindi pa rin natin sila masisisi dahil klaro naman na wala talaga silang naintindihan sa sakit ng kanyang misis. Nakakalungkot lang kawawa ang nurse kasi siya yung sinisisi. Kung naeducate lang sila ng tama, baka di pa umabot sa ganito.

Kaya importante talaga ang pagestablish ng rapport and EXPLAIN every procedure na gagawin sa pasyente

from 2018 I have 4th stage of cancer from lungs and its already spreads from all over of my body. Labas Pasok ako sa hospital dito sa Luxembourg/Europe but we never pay this huge amount bill in the hospital, grabe mas
mahal pa sa Europe at wala pang kasiguruhan ang pag galing mo.
Lahat ng ibinayad namin sa hospital ay binabalik sa amin at kahit ang transportation ko between Home and Hospital ay libre. Sana naman mabago na ang mga maling patakaran ng Health Care diyan sa atin . You just working so hard to pay your bill in the hospital. Mabuti na lang di ako pumayag na magpagamot dyan sa Pinas. God bless to evryone and be strong to All who are sick. Eat a lot of green vegetables and Driink Vitamin C and a lot of prayers.

My condolences to the family.

I am a doctor and at the same time a nurse, I agree na sensitive since may cancer si patient and knowing na nag chemotherapy si patient. With that, prone talaga si patient sa infection especially sepsis since immunocompromised or mababa ang immune system nya. Meaning prone talaga sya sa infection

Kung ibe-base sa kwento ng relative, lalabas talaga na may mali. But then again, I think walang pagkakamali na nangyari dito, walang medical malpractice na nangyari. Prone talaga pumutok ugat ng patient, since nag chemo nga sya, also mahirap talaga tusukan. I don’t think na dahil sa pag insert ng IV yung naging cause ng sepsis.

Wala rin naman nagsisimula na magaling, syempre mag uumpisa ka sa mababa. For sure yung mga naging doktor ni patient, nag umpisa rin yan sa mababa at hindi pa ganun kagaling, mahuhubog lang yan kapag marami na experience. Hindi lang naman sa medical field, sa other profession din naman.

So sa tingin nyo ba yung 1 year old child marunong na mag drive ng kotse?

Kaya hindi natin masisisi rin na maraming umaalis na nurse sa Pinas kasi sobrang unappreciated sila.

Ive been a nurse in the US for 20 years and 5 years at PGH, my take is that patient already has compromised immune system from CA, shes very susceptible to secondary infection. The iv itself is not to blame, plus the patient probably took her vent off, as the usual case, dont blame the nurse!
