Tetraphobia also applies to the character, Four, the host from Battle For BFDI. Not only that Four can Screech you so loudly that you get knocked out, but he can also morph matter, and zap you to death. What a truly terrifying character.

Sounds like the fear of someone who has an m.44 pistol with living beings that ride on the bullets that come out of the gun, he also hates the number 4

In Korea, some of the elevator buttons for 4th floor say 'F' instead of '4' for this reason.
I guess some people do really fear the number 4, or it is that elevator companies fear someone with tetraphobia might complain about it...
