What classifies something as a "mod," and what are the different types of said modifications?

I modded my skyrim so that all the dragons were trains and any time you passed a skeleton, it would play "spooky scary skeletons"
Edit:specifically trains from "Thomas the tank engine"

I was watching a playthrough of heavily modded oblivion. It was modded so heavily, after like 6 or 7 iterations of the mod set you couldn't travel to the western half of cyrodil without instantly crashing

HUD/UI mods are something I really geek out on. Especially ones that combat games that don't upscale well over the years.
