Itachi getting a weird disease isn't too out of reach. As a young prodige, he was present in many battlefields during the 3rd Shinobi War, where after having won a battle he would hang around the fields looted with thousands of decaying bodies, and in true young Itachi fashion, he would probably stand there as he poundered about the harshness of life lol
