Discharge to recover then assess (D2RA): Pathway 1

Developed by the National Six Goals Programme for Urgent and Emergency Care, the Discharge to Recover then Assess (D2RA) model is based on the principles of effective and timely patient discharge home. It has recently been revised to ensure that healthcare practitioners focus on ‘what matters’ to each individual patient when it comes to discharge, in order to achieve the best possible outcome for recovery.

The D2RA model features four different patient pathways. These include:
- Pathway 0 – No additional support required for discharge
- Pathway 1 – Supported Home First
- Pathway 2 – Short-term supported facility
- Pathway 3 – Complex support

A series of short animations have been developed to increase understanding of each pathway and to support with the embedding of the revised D2RA model across NHS Wales.
