Cool concepts that'll never happen:
When you go to an underwater temple, the Elder Guardian doesn't have a health bar, and when you defeat it, the Leviathan appears.

Mobs change into their counterpart in Ultrakill

Zombies have a chance to spawn as Filths or Schisms

Skeletons have a chance to spawn as Strays or Soldiers

Spiders have a chance to become Malicious Faces and an incredibly low chance to become Hideous Mass'

Phantoms have a chance to spawn as Drones or Mindflayers

And, at the end of the game, Gabriel kills the Ender Dragon, and you have to either kill his first version, his second one, or one after another
