NAEMT's Prehospital Trauma Life Support (PHTLS) is recognized around the world as the leading continuing education program for prehospital emergency trauma care. The mission of PHTLS is to promote excellence in trauma patient management by all providers involved in the delivery of prehospital care. PHTLS is developed by NAEMT in cooperation with the American College of Surgeons' Committee on Trauma. The Committee provides the medical direction and content oversight for the PHTLS program.

PHTLS courses improve the quality of trauma care and decrease mortality. The program is based on a philosophy stressing the treatment of the multi-system trauma patient as a unique entity with specific needs. PHTLS promotes critical thinking as the foundation for providing quality care. It is based on the belief that, given a good fund of knowledge and key principles, EMS practitioners are capable of making reasoned decisions regarding patient care. The course utilizes the internationally recognized PHTLS textbook and covers the following topics:

Physiology of life and death
Scene assessment
Patient assessment
Airway
Breathing, ventilation and oxygenation
Circulation, hemorrhage and shock
Patients with disabilities
Patient simulations
PHTLS is the global gold standard in prehospital trauma education and is taught in 64 countries. PHTLS is appropriate for EMTs, paramedics, nurses, physician assistants, physicians, and other prehospital providers. PHTLS is accredited by CAPCE and recognized by NREMT.

My take for this Trauma Assessment

General Impression is missed the moment you approach the patient you must already verbalize the status of the pt, awake, purposeful movement, any obvious bleeding.

In PHTLS Application of SMR (Spinal Motion Restriction) has criterias
1. Know the MOI if it involves Fall from certian heights, roll over crashes, collisions with obvious damage on the vehicle.

2. Pt must be reliable for information meaning pt is awake, no loss of consciousness, not intoxicated.
3. Proper neurological assessment.
In this case the patient was hit by power saw, no Head Injury noted and was not invloved. Patient is awake and responsive.
So SMR is not applicable, no need to put patient on C collar and Spine board.

Primary Assessment Circulation
In PHTLS the goal is to quickly pack the patient for transport and application of IV is not recommended on scene. It must be during transport as to save time.
Also during the E or Expose dont forget to place thermal blanket as this is the triad of death “Hypothermia” plus the profuse bleeding “Coagulopathy”.

On the secondary assessment getting the body temperature of the patient is of vital information. The instructor should have mentioned this info as this will be baseline for checking hypothermia.

The tourniquet should be applied as upper as possible in the extremity and not just above the wound

Sir, question. Is it only for retdem purposes that you did not apply the c collar after you and your partner approached the patient?
