If I'm understanding the presentation of this non physician, he is making a clear recommendation for alteration of dx. and tx. policies based on his conclusion that a small percentage of men with high grade tumors will die of PCa in the near future. That is essentially "why diagnose 150 men with high grade tumors to save 5?". I'm not sure I fully comprehended his thesis because some of his words were obscured and his narrative style is that of an academe rather than a physician, but I'm completely unconvinced that his word parsing conclusion that targeted MRI leads to overdiagnosis (as opposed to mistaken dx) and overtreatment of high grade PCa.
