Don't Ignore the PUA Documentation Email. If you fail to submit, or if you don't have the right documentation, you may be liable to pay back all of your ineligible PUA/Unemployment. Or, as some may call it...a loan. You have 90 days from the time they notify you.

My employer is refusing to send me a copy of my 1099. I have emailed texted and called them, and keep getting excuses from them.
I got a lot of money from the state (and had to use it for bills) i dont have that money anymore I am so scared.

I received a letter in the mail two weeks ago saying that I had to provide documentation. Even though I already did in April online. They said they want the documentation before June 1st. I don’t know what else they want.

Did you get any latter from PUA after you send proof of work? I got one that send to me the latter look the same that they send email to me. What should I do? I already send w2 to them I didn’t see any email PUA on my email.

hey bro man thanks for this video so i got the letter but when i click on the link i get no submission portal instead i get redirected to a page that reads this : You are not an authorized user of the PUA Eligibility System. If you received an email from NYSDOL asking that you provide documentation, you must use the account associated with your PUA claim. Please go back and verify that you are logging in with the NY.gov account associated with your PUA claim. If you did not receive an email from NYSDOL, then you are not an authorized user. You will be redirected momentarily. any idea whats going on?

What year of w2 do we need to send in? 2020 or 2019?? And do we send screenshots of the w2? Or do you upload files?

Does anyone know how to find the form? The link in my letter does not work. It says I am not an authorized user of the PUA system. I’ve been trying now for about four hours to find out how to get this form. Nothing works. I couldn’t get through to a person either. Thank you so much.
