In this short video Dr. Om J Lakhani talks about how to calculate insulin dose in hospitalised patients with diabetes

This informative video provides a comprehensive guide on how to calculate the insulin dose for hospitalized patients. It covers three different scenarios: patients transitioning from ICU to ward, patients already on insulin, and patients starting insulin for the first time.

Transitioning from ICU to Ward: The video explains how to calculate the insulin dose for patients transitioning from ICU to ward. The calculation is based on the amount of IV insulin the patient was receiving in the ICU. The total dose of insulin received in 24 hours is divided into 50% basal and 50% bolus insulin. The bolus insulin is further divided into three doses to be given before each meal.

Patients Already on Insulin: For patients who were already on insulin before hospitalization, the total insulin dose they were taking is calculated. This dose is then divided into basal and bolus doses based on the type of insulin they were taking.

Starting Insulin for the First Time: For patients starting insulin for the first time, a formula is used to calculate the starting dose of basal insulin. The formula is: (fasting blood sugar - 50) / 10. The total basal dose is then divided equally into bolus doses to be given before each meal.

The video also emphasizes the importance of adjusting the insulin dose based on the patient's blood sugar levels before meals. This is a crucial step in ensuring effective blood sugar control in hospitalized patients.

This video is a valuable resource for healthcare professionals looking to understand the process of insulin dose calculation in a hospital setting. It provides practical examples and clear instructions, making it easy to understand and apply in real-world scenarios.

Sir can we use 0.5 × body wt. = total requirement of insulin/ day and then divide into basal and bolus dose?

Suppose the patient received 10 units of insulin infusion in last 6 hours in ICU . Then we have to consider it 1.66 unit/hr converting to 1.66×24--- >40 units to consider total requirements in 24 hours ?
