David Bougher, RN, BSN, SVP of Regulatory Affairs, and Deanna Rice, Pharm. D. VP of Clinical Services at WISE Hospice, teams up with KanTime to discuss deprescribing.
Deprescribing has been a component of hospice care for over 35 years.
In recent years deprescribing has become more formalized, backed by research and clinical practice, and adopted as a standard of practice in other medical specialties such as cardiology and geriatrics. Considering deprescribing is essential when providing care to the frail elderly at the end of life.
This session will provide a basic introduction to the practice of deprescribing. We will look at definitions of and rationales for deprescribing.
In addition, we will give a brief overview of the categories of medications that should be considered for deprescribing in this population of patients.
We will finish the session with some strategies for discussing deprescribing with patients and families, and your professional colleagues.
