My esr is 38 mm/hr . Recently i had very much fever and infection . Please tell should i be worried about esr . I still have sometimes pain in stomach and rashes like mosquito bites on stomach, hands and legs . My tlc was 14.6 thou/mm3 and lymphocytes dlc was 16% and platelet count 400thou/mm3. Can you identify my problem??? Please help.

Requirements

1. Materials:

Westergren or Wintrobe tube.

Westergren stand or rack.

Timer or stopwatch.

Pipette or syringe.



2. Reagents:

Anticoagulant: Sodium citrate (for Westergren) or EDTA (for Wintrobe).



3. Blood sample:

Fresh venous blood.



4. Environment:

Room temperature (18–25°C).

Vibration-free surface.

Principle

When anticoagulated blood is left undisturbed in a vertical tube, red blood cells (RBCs) settle at the bottom due to gravity.

Sedimentation Rate: Aggregated RBCs settle faster than individual cells.

Influencing Factors: Plasma proteins (e.g., fibrinogen, globulins), RBC shape, and count.

ESR reflects the presence of inflammation, as inflammatory proteins (like fibrinogen) promote RBC aggregation.

Methods

1. Westergren Method

Equipment:

Westergren tube (200 mm long, 2.5 mm diameter).

Sodium citrate as an anticoagulant.


Procedure:

1. Collect 4 parts of blood in a tube containing 1 part sodium citrate (anticoagulant).


2. Mix the blood gently but thoroughly to avoid clotting.


3. Fill the Westergren tube up to the 0 mm mark with the anticoagulated blood.


4. Place the tube vertically in a Westergren stand without tilting.


5. Leave undisturbed for 1 hour at room temperature.


6. After 1 hour, measure the distance in millimeters between the top of the plasma and the top of the RBC column.



Normal ESR Values (Westergren method):

Men: 0–15 mm/hr.

Women: 0–20 mm/hr.

Children: 0–10 mm/hr.

2. Wintrobe Method

Equipment:

Wintrobe tube (100 mm long, narrower diameter).

EDTA as an anticoagulant.


Procedure:

1. Collect fresh blood in a tube containing EDTA.


2. Mix the blood thoroughly with the anticoagulant.


3. Fill the Wintrobe tube up to the 0 mm mark.


4. Place the tube vertically in a stand without tilting.


5. Allow the blood to settle undisturbed for 1 hour.


6. Measure the distance in millimeters from the top of the plasma to the RBC column after 1 hour.



Normal ESR Values (Wintrobe method):

Men: 0–9 mm/hr.

Women: 0–20 mm/hr.

Factors Affecting ESR

1. Increased ESR:

Infections, inflammation, anemia, autoimmune disorders, cancers.



2. Decreased ESR:

Polycythemia, sickle cell anemia, hyperviscosity syndromes.

Blood sample should be fresh.
