Mitral Regurgitation- moderate

Note the holosystolic high pitched murmur, which does not vary in loudness despite the irregular cycle lengths; this classic finding proves the murmur is from regurgitation, not from ejection, and can easily distinguish the murmurs of aortic stenosis and mitral regurgitation.
