Gameplay wise, the dimensions aren't connected but it makes sense. You aren't supposed to go past the bedrock in any dimension so in theory they could be connected.

Steve doesnt have amnesia, rather minecraft is a simulation. The "players" instantly appear in a random place in the world. The new minecrfat updates, are just updated versions of the simulation. Also notice how with certain updates, the game becomes more like real life (aquatic update, caves and cliffs, etc.)
