How do you draw the perfect size of the traits

I don’t understand how you got the layers to coordinate at the correct spots. How do you get the horns to go on top of their head, mouth go in the right place? Etc

Can I link layers together? For instance, 1) if a character is throwing up linking the throw up with an open mouth OR 2) two hands but one in the foreground and the other in the background that turn on if the other is present?
