one thing i saw people doing in a coop run was they'd roll the dice emote and whoever rolled the highest took the item

Luck works as follows in co-op:

Personal effects (such as Holy Light or Tough Love) are based off of the player that holds them's stats. (If Isaac has 9 luck and Tough Love, he will always have teeth shot, even if T. Lazarus has -302 luck)

General luck-based things (such as room clear rewards) are based off the sum of all of the player's stats. For example, a Keeper, an Isaac, an Eden with +0.5 luck and a Cain would act like they had -0.5 luck at the start of a run (-2 + 0 + 0.5 +1 = -0.5)

Btw a really nice tip to pull the trick of grabbing both items in alt floors is to position yourselves normally against the walll and then set a time using chat referring to the in game timer to both run at the same time (Eg press "W" at :15:35), it worked most of the times I tried it with my friend even with horrible ping :)
