Okay, Brandon, to cause a nuclear melt down, you must superheat the core of the reacter.
This will sound complicated because I'm no nuclear engineer but I did study nuclear power/accidents, but I'll try to explain the best I can.

First of all, we must understand that the core only heats up because of a natural phenomenon known as "nuclear fission." Nuclear fission is the process in which the nucleus will split spontaneously when hitting another particle releasing great amounts of energy in the form of both heat and radioactivity. That's how it heats itself up. We should also know that the core is typically full of fuel rods (usually comprised of U-235), which are actually multiple small tablets in one tube so the particles can split the nucleus.

Now to prevent the core from superheating or "melting down, " control rods that act as a sponge must be lowered and absorb the energy being released from the fuel rods. And if the core does meltdown, it's literally melting down because the fuel will become so hot that the core will begin to melt into a lava-like substance known as "corium."

Another thing is that the reactor is often deep underground and in great distance of the "smoke stacks" so you may need something with great impact, Fukishima did have a melt down to a stunami. But keep in mind not all meltdowns show a mushroom cloud.
